I think it should work like the tribal mans shiv (sniper item) and the Flying Guitine. (scout item) It should be throwable causing bleed for 6 seconds and whenever you do a regular stab it should do 6 seconds of bleed as well. Each second causing 2 damage. As for a penalty, -25 health on user and -20 percent cloak duration.
