Xaphan's item won't actually kill you from spamming. His item depletes charge whenever you would take a blow that would normally kill you, so I assume that's what happened.

I meant the fact that i still had 1/2 of a heart when i was dead.

That's part of the item. If the item loses all charge, it will kill you. I probably will change it so that the item just disappears when it reaches zero charge to avoid confusion.
