Really Weird Bug: Tab
Programmer to programmer... this one is weird.

I just noticed that if I hit tab while firing... the radius of the tear orbit slowly increases.

It'll stay at the new radius until I move, then resets down to normal.

Her orbit growing is part of her design to ensure she can hit things that Isaac's velocity based tear movement can. You don't need to hold tab, though.
