Your suggestion is a good one, and it will be implemented, but due to the latest update (tonight) it can't be implemented yet BECAUSE... after tonights update is released there won't be a fail rate because it won't fail anymore. :P
Tonight's update fixes the buffer overflow error which was why there is a fail message. It wasn't an intended mechanic but more of a bandaid until we could get item generation working as intended.
HOWEVER. We do want to add the ability for item upgrades to fail and consume a portion of the resources that were supposed to be used to upgrade with the fail rate being configurable via INI.
How does that sound?
Also, I would also like to add my "yes" vote to the "Fail Rate" .ini option. This, also, can be turned off by server admins by setting the "Fail Rate" to 0.
Regarding the "Fail Rate" option, absolutely. We're hoping to add configurable options for most of what the mod does so players can suit it to their individual play styles.
