Can you fix it?
This has happened to me before, but just go into console and type "record 1", press enter, and then type "stop". What this does is it re-renders the textures to prepare for a recording, but stops it after a few seconds (when you type "stop"). If you do this a lot and it starts taking a lot of space on your computer, when you stop it, it should tell you where the recording was saved to (in console). There, you can delete all the recordings and then start anew and use it as a troubleshooting method again.

You're right, this happens in all sorts of gamemodes. To list a few, TTT, Morbus, and even sandbox. All it needs is the record troubleshoot (record 1, stop) and bam badda bing, it's golden. Though I've noticed it happens alot in HomFixed, it isn't really too much a problem.
Edit: Oh, right, now I remember. I disabled a feature that fixed that bug. It is a bug in the base game. I can re-enable it as soon as I can.
