Although there is also a slight chance that Hide Weapons in Animations might cause it, since it fiddles with animations and cutscenes trigger animations! Sounds like a weird logic to follow but once you've been modding this game for a decade you'll understand its never been very mod friendly like, say, an elder scrolls game.
In conclusion: I suggest you try removing the Hide Weapons first and just load a save from before you entered the tomb (this is easier to test). If it still crashes your game then all thats left is M4-78.
M4-78 is trickier to test. You would have to load a save from BEFORE you first enter Korriban (to avoid any save corruption, since you would be loading triggers and data that dont exist anymore).
