Each level has a book you need to find, marked by a red sigil. The first level's book is on the roof in a dumpster, the second level's is in the red tunnel on the left hand side, and the third level's is under the jukebox in the room with the negev dude

You need to get all three, and then go to the room after executing the guy at the end of the third level. If you die at any time during this you have to restart and try again from the start.
