Good idea.
He braces himself, then shoves the grip forward, which makes the head spin and generate team-colored lightning. He then shoves it forward. Anyone killed is gibbed and disintegrated, a la cow mangler. He maniacally laughs while doing so (like when he gets a rare spell)
Considering that in TFLore, Tesla was the original Engineer, maybe it could work off the fact that it was an invention by Tesla that was created for Sigmund Freud, who was the original Medic in TFLore.
For a taunt kill, I thought of the Medic stopping, and examining his wrist. As it's an old design, and it could be possible that he just found it, he would have no idea what it does, except for hitting people. Maybe theres some buttons on it? If so, then this would happen.
Medic stops short, and examines the conductors.
He mutters to himself, maybe something like "Hmm, let me see here" or "ooh, a button!", which he immediately presses.
This results in an electric shock, and he could possibly enter an animation of him contorting in mid air, like in the old cartoons when they stuck their fingers in a wall-socket.
Once it is finished electrocuting him, he would stop his contortions and land on the ground, woozily bobbing to and fro, before shaking it off and continuing on.
But, though the electrocution would only shock the Medic, it could electrocute the enemies around him/in front of him.
It could work like the Rainblower taunt for Pyro, where it only kills about 3 people or so. The death animation would be, like people before have mentioned, like the death animation from being killed with the Cow Mangler/ Righteous Bison/ Neon Annihilator.
That's my idea for the taunt kill.
If that doesn't suit, then perhaps if you just wanted a taunt, it could play out like the taunt kill I talked about above.
Medic stops, and examines his wrist, and upon seeing a button, he could possibly say "Hmm, let me see here" or "Ooh, a button!". After saying "Ooh, a button", he could press it, and it starts to charge, which makes him say "Oh, no" or "Oh dear", before it fries him.
That could work for the taunt kill as well.
For the dialogue "Hmm, let me see here", comes from the end of Meet the Medic, when you hear him finishing up on Scout, before it ends.
A few other people have had some really good ideas for the taunt-kill.
