I quit out of my save to check if I have a mod installed. Knowing I do, I go back in. For some reason my fanny pack turned into the colorful one... Then I notice I have a shiv back, and another fanny pack... And a spoon, and the new pack has the same items like the previous pack.

tl;dr, Mod gives you Inmate starting items when loading back into the save.
