I have had one vanish on me a couple of times but is difficult to replicate, usually after changing floors then exit a building - but - entering a vehicle while they are follow you can trigger the enter vehicle command so they reappear.

I would also recommend logging out and make manual save backups every so often - this is just because, well b42 and all that
