I've only one secret missing but I don't know where to look for...
I can tell you there are 7 secrets in the Pale Moon Catacombs (including the 2 at the beginning of the Path of the Conduit). In the Hall of Correction, after the Golem Boss, you can go back to the Burning Chamber to grab some items but no secret activated.
It's actually a long while since I last played this mod, but I still never was able to find the last three missing secrets. I still do have some of the secrets written down here in detail:
Pale Moon Waterworks Underbelly: (1 Secret)
(Button -> Shield, Crystal Plant)
Pale Moon Waterworks: (2 Secrets)
- Button after diving, Leather Armor, Bow, Arrow
- Button on wall after Ratling Trio, Leather Boots, Note)
Pale Moon Castle Grounds: (1 Secret)
- Next to Mansion, Miner Note, Bomb, Pickaxe
Pale Moon Catacombs (3 Secrets)
- Northwest, Button on Wall, 2 Potion Chamber
- Middle, Button Wall - Helmet
- East, Ammo, Ring Mail Chest
Pale Moon Castle (3 Secrets)
- Pressure Plate, Bridge, Lock Pick, Archmage clothes
- Underwater Lever, Scroll of Lightning Bolt
- Halls of Wisdom -> Opposite Halls of Illusion
Halls of Correction (3 Secrets)
- Secret button after entrance, Cannon, Ammo, Plate Chest, Herbs
- Cells of Conviction, Tome of Knowledge, Willpower Potion

Just loaded a save and yeah, it's still 19/22 ... any idea where the other three could be?
I vaguely recall some secret with a Lightning Blade, but maybe that was another mod or I misremembered ...
- Just where you found the Helmet : there's a secret button and another cache (a pie and a crystal flower, IIRC). Yes, a secret in the secret !
- On the Ring Mail chest segment, there's an altar where you found an ornate (?) key and fought 2 skeleton warriors ( after 2 skeleton archers). On the opposite wall, there's a button for a hidden rifle and other stuff.
I miss only one secret and I'm pretty sure there's the Shield of Valor inside (all other pieces can be found).
