i feel like this dude should add spearmaster spears from rainworld. they should just drain health slowly. sadly, i have no coding knowledge. the best i can do is predict simple stuff like checking if the players health is specifically at 50. also i don't know how to mod. (mostly because i usually don't use anything that requires downloading, except from sources i trust, like steam.) so either someone else can make it, or this guy can.
