Since poison headcrabs can't kill their targets, if you don't have the option for headcrabs to always instantly take over suitable hosts, they will instead have a random chance to take over an enemy every time they bite it. The target's health shouldn't affect this process the way you described it, so it's almost definitely some sort of mod conflict.
