Some are an instant shield overcharge, but I think the others just give weapons to the passenger. If they do anything directly, the other people replying don't know how to activate them either.

From what it seems, all of the "items" in the mario-kart levels are either healing, over shields, or giving Johnson different weapons. Im pretty sure that the Blue Shells are actually unavailable to the player, and that they only spawn when the player is in first place. I know this because if you stay in 2nd place the entire race, the AI wont be hit with a Blue Shell, but the moment you take first there seems to be a countdown until you get hit by one.
