Only these 2 mannequins (with a smiley from the roof of the building and with an eye from the bunker) are different from a bunch of others that you can find all over the map (in the building and the bunker)

In the basement, you can die if you enter a room with a scary face before the script breaks the boards to this room (the script is triggered when you start to leave the basement). You die by teleporting into a dark room and hearing the "voice of the monster that killed you"

the eyes mean something for sure could it mean something is watching you?
or the ash pit is watching you
