You have to press H on the small bit to pull the visor across

sa stands for "Special Activation" or atleast i think

for that its a face shield it attachs on its own no limb cutting required grab it while its attached and press H and it will turn on the shield which has limited durability it blocks bullets from certain ranges (i like to break it and use it like a cyber jaw)

It Happens For Me Too, But Just the Arm Bit.
