I can't seem to get to the tomb floor. I've gone through glacier multiple times with no luck. The hub trapdoor also doesn't spawn past the first floor for me, and only takes me to glacier and downpour.

I am on a relatively new file. Is there a prerequisite that needs to be met before it is unlocked? I have tested it up to unlocking the repentance paths, and still nothing.
