The 1.28 update changes how mods are uploaded and downloaded to the Steam Workshop. They are now uploaded to a folder within your Steam install folder for workshop content. This is so that mods are less likely to be tinkered with and it doesn't clutter the mod folder for mods that modders are working on.
The EU4 game has a unique number in Steam, that number is: 236850.
The SuperStates Mod has a unique number in the Steam Workshop, that number is: 284344558.
To play the mod after the 1.28 Update, you must first roll back EU4 to 1.21 as normal from the instructions above.
After the game is rolled back, you must search for the folder named 284344558, it will be found in the folder chain "...\Steam\steamapps\workshop\content\236850". Open the folder named 284344558 and there should be a file named *_legacy.bin, this file is a standard *.zip format with a different extension. You need to copy this to the standard mod folder in "...\Documents\Paradox Interactive\Europa Univeralis IV\mod\" AND rename the file and its extension to superstates.zip.*** After that open superstates.zip in any zip file extractor and copy and extract the file descriptor.mod and save it to the mod folder and rename this file to superstates.mod. Once this is done, it will allow you to play the mod in 1.21 until it is updated.
***If your computer has *.bin file extensions assigned to a program, you may need to enable viewing of file extension in order to change them. To do that open Viewing Options in Windows File Explorer and check show file extensions.
