If you took a look at the item in game (which I highly recommend you do, there is a listing for the item on mods.tf that you can download and try very easily) it is not made with that intention in mind. In fact, it looks perfectly fine if not pretty damn good on a relaxed, hippie, skater-style scout. It's punk, rock-and-roll, especially if you combine it with the right gear. It matches a scrawny, womanizing sleaze who's confident enough in his ego to show his midsection without much detail, and goes about it without any semblance of self-awareness.
However
I think I know where this is coming from, and I have said it many times before in the comments of this item and in the other discussions: the people who obsessively type "FEMBOY SCOUT OMG SO GAY SO BI SO GAY I WANT TO HAVE SEX WITH THIS FICTIONAL CHARACTER GAY GAY GAY" completely and utterly change the tone of this item. I also personally believe that the first thumbnail for the item and its pose doesn't help to mitigate that kind of behavior, along with some comments made outside of these forums, but I digress.
I would, again, HIGHLY recommend you give this item a try in-game first, and I believe you'll see what I mean when I say this works, and that the only impression of this being something with a perverted inspiration comes from the idiots who think the words "sex" and "femboy" are the only punchlines on this spinning rock.

The spy-maid outfit, which suffers the same issue as this cosmetic (having a gang of porn-addicted miscreants commenting repeatedly on how much it aligns with their gross, weird, and inappropriate behavior) is more or less a cross-dressing joke, like the ones soldiers used to do in the army.

Of course, brain-dead coomers can't get a joke and instantly fetishise anything they see like the pack of terminally online social dropouts they are.

Give this cosmetic a chance, separate from all of the noise. I promise you, it's not as bad as you think and not as egregiously sexual as these psychos make it seem.
