You need a huge city in one of the north Italian cities.

YES thank you ! I want to keep this discussion here because I always thought it was a yearly trigger like M2TW. I finally got a huge city and am promptly converting my legions

Does this include Rome itself? Or does it have to be another of the cities?

If I remember correctly, it actually EXCLUDES Rome - the hardcoded trigger is any city in Italy other than Rome reaching Huge City, but because the mod changes so much of southern Italy, that region is bugged, which is why Komisarek says "north Italian cities"
-Arretium, Ariminum, Patavium, Mediolanium or Segesta....fyi, because of growth rates and farm land, Patavium is the easiest/fastest to get that to happen
