I suspect there was a change with 1.6 or a recent update that isn’t playing well and causes a lot of logic to rerun unnecessarily, but I don’t have a lot of leads at the moment.

If there's any further logging or testing I can provide from my setup, or a video/etc, I can do that.
Narrowing it down to Map Designer may have been a chance problem as I don't see how these two mods interact whatsoever, however my binary search and eliminate of my modlist (~80) resulted in Lights Out and Map Designer both being active as the only case where it occurred.
Removing Map Designer but leaving in Lights Out along with the other mods made it run fine. I just don't get the correlation.
The hitching still occurs with only LightsOut, but it's miniscule that you don't really notice unless you're looking for it.
The more mods you add the more the worse it gets and the longer the culprit functions take to execute.
Check the Harmony Patches in perf analyzer, specifically look for the "Max execution time" column in perf analyzer.
EDIT: I wasn't seeing this hitching last week, I found it odd it was suddenly occuring, so likely a RimWorld update?
