If you search "GM13" in the search bar for the tools, you can find the seamless portal tool gun and what not. I accidentally stumbled upon it looking for my RT camera.

I found these probably thanks to Wiremod's custom tool menu (probably not what it is actually called). Or extended spawnmenu addon (useful if you wanna cheat and see the secret texture or hear the secret sound files of this map). Anyway, they appeared at the very bottom on Tools menu
