Gave it a quick run-through and found out that if you get killed by a Charger or Jockey via special attack then the game still thinks you're incapped even though you're dead.
Also if you respawn while this is happening, you can't move and you still take damage from whichever killed you. Killing or removing the npc will fix this.

Also doesn't seem to be a bug but why do AI Smokers just let you go after a bit if they grab you?

Update: The issue with the Charger and Jockey also happens with Smokers.
The Hunters don't have this issue, but after they kill you they get teleported to a specific spot on the map. (This was tested on Construct and they'd teleport infront of the base at spawn)

When I first got this I already had cpt hazamas so they over laped so disabled them to check out these and there errors

I assume that sense I ♥♥♥♥♥♥ up I now rely on the other ones I'm going to restart my computer and see if that works

Hmm, me and Tensor never experienced the issue, maybe check if you have any conflicting addons or too many addons

When the Charger kills an NPC with it's special ability while Keep Corpses is on the NPC is frozen in place with the last animation it played, for example a metropolice is just standing in place while pointing an invisible gun, their collisions dont exist either.

i have some bugs: when the smoker grabs someone u can see like a gravity ball under your character, also, when u using like a NPC with a grabber special ((example: hunter)) then there appears like uuhm, the original NPC standing, i mean: there is 2 npcs

The witch has a random habit of crashing with certain NPC Mods.
