While I can answer most of the questions you have in this post, I am not the main developer of Vanguards of the Lost Lands and will ensure that anything I can not answer satisfactorily is brought to the game designer, resolved, and the rulebook is changed to implement these answers in a clear manner for players in the future.
When a hero dies that hero is discarded from the game and the player is allowed to play as a new hero of their choice or at random. All items, buffs, improvements, and followers are lost upon death. Currently, we have no possibility for hero's to be revived as the player would now be playing a new hero though, we can certainly look into that possibility as an alternative to playing a new hero. Yes, hero's can die outside of the battle with the Villain and the way that we have been play testing the game we have allowed for players to heal at the end of the combat round while at full injuries and die if they can not do so. Followers are discarded upon death and, while the option of healing a follower has not come up in play tests I have been involved in, I will be sure to talk about that option with our game designer. Lastly, while yes it is possible for a hero to die near the start of the game, due to lack of honour to heal, it has been our experience in play testing, both internally and externally, that it is a rare and unlikely occurrence that does not overly affect the game in a negative way. However, that being said, we will be sure to keep an eye on it and will make changes as necessary to keep that experience to a minimum.
I hope this answers the majority of the question raised in this post. It was mentioned that there are currently other mechanics that raise similar such questions and we would love to know what those mechanics are so we can make sure we look into and clarify them for future players. Again, thank you for taking the time to write this detailed post. We very much appreciate everyone's efforts in making Vanguards of the Lost Lands a better experience for our future players.
I will do my best to answer your questions directly and would love to hear every concern or question you have and answer them to the best of my ability.
We plan on relaunching the Kickstarter sometime later and simply as a board game. This decision comes after quite a bit of feedback and discussions based on what is reasonably feasible for the project.
What I am going to do is address each point and add to Death_Warden's comments.
*What occurs when a Hero actually dies? If a Hero maxes out on injuries, are they discarded from the game?
-Yes once a hero reaches full injuries, if they cannot heal themselves then they are discarded from the game.
*If a Hero dies, does this mean the Player is eliminated, or do they draw another Hero and try again? Would the new Hero retain any items or stat improvements from the previous Hero?
-In this case the player is NOT eliminated, rather they can choose or randomize a new hero to play, however all items and honour of the previous hero are lost.
*Can a Hero be revived, and if so, when, and with how much health, and at what location?
-Currently heroes cannot be revived, however if a new hero is chosen after a player's hero has died, they always start in the village.
*Can a Hero actually die outside of a conflict with the Villain, or is death a mechanic reserved specifically for this encounter? (Several high-profile games reserve death and elimination solely for the final encounter.)
-This is the one situation where player elimination takes place. During the final encounter with the villain player death means elimination from the game.
*Can a Hero spend 1 Honor to heal from maximum injuries after the combat round when they have received maximum injuries, or are they irrevocably dead as soon as they receive their final injury?
-Yes, if a player has honour, they can always heal their character.
*If a follower dies, is he discarded? If so, is he discarded immediately or can he be healed with Honor at the end of a round similarly to the Player's Hero?
-If a follower reaches maximum injuries they are discarded unless the player heals their follower.
