It's in a room with a large pit where the goal of the puzzle is to align a bridge using a wheel to spin it and then jump on the rotating bridge.

At the bottom of the large pit (use the ladder to go down to the bottom of the pit or you will be hurt), hidden behind the pillars is a skull. Pick it up and you will catch fire. Even with 100 health if you do not use the endless medkits at the top of the pit, the fire will eventually kill you.

I used the skull to kill the hidden sleeping houndeye. Does that make it further cursed?
