About LeftBehind, i've seen it when there was a dead player ready to be rescued from the closet but the teammates ignored him.
Left4Dead, for me, triggers every time all the survivors are incapped or dead and the chapter is about to restart.
for the "Left4Dead" Fault, I did mean that it triggered for chapter restarts from being overwhelmed, maybe I should’ve been more clear
for the "Reckless" Fault, it has triggered once for me when a bot tried to move through my line of fire when continuously shooting, although it does not count if bots teleport to you and probably works better with players
I’ve never seen "LeftBehind" since I never abandon rescue closets and neither do the bots (I did this testing on Expert as it is the difficulty I am comfortable with)
It’s interesting to think what the original intent of the fault system was, maybe it punishes people from their faults? Although that doesn’t seem to be that case or if it is, it’s a very minor effect.
The "LeftBehind" Fault is indeed activated from abandoning nearby rescue closets
And I have also found a new fault "Gotcha", I have no idea what this means but all I know is that it has appeared once during the first chapter of Dark Carnival, outside the saferoom.
One of the bots was being overwhelmed while at the saferoom door, attempting to open it while surrounded by a horde, there was also a hunter nearby, it appeared right before they got scratched by the hunter although I don't believe that is related.
This was found during an Realism Expert run although I do not think the game mode will matter
