Probably lightning?
After a fall guy dies, he usually changes color and almost always a black square appears under him, are you guys getting this bug too?

I'm guessing it's because you played online with the mod, with your gore settings on "High", I'd recommend changing it to "Low" to see if it fixes it.

It also happens if you kill them with fire, even with "Low" gore.
