I think my general theory was/is that The Institute finds and refurbishes existing Dolls. They need to inspect the player in order to learn enough to fix one of their currently-disabled Dolls. Lore-wise, the Institute has a secret location with a warehouse filled with deactivated Dolls, but this has not been implemented in the game. Maybe they stumbled upon it at some point.
