Strange character age warping
There is a strange bug Im not sure if its the base game or not but the ages of the characters keep on change back and forth jumping from like 19 to 18 to 21 and back to 18.

It happens when I play on the 7ad start date all the leaders and their heirs are changing their ages back and forth like from 18 to 23 to 18 again. is this a base game bug or is it a mod issue?

A game issue. The game is not designed to handle negative dates, but starting in 7AD many rulers would have their birth date in negative dates so the game can't handle them properly.
