I replied to your comment but I didn't notice the thread until now. Anyway I might as well answer this here in the event of someone else having the same problem in the future, and god forbid you'd be able to find solutions in an addon's main comment section.
The cause of this happening is an areaportal did not open properly, and it's probably because a door was either deleted or moved. Areaportals are used by mappers to optimize maps USUALLY by placing them inside door frames and window frames but can also be used to section parts of maps, this is probably why you almost consistently see this issue in door frames whenever it happens. 3kliksphilip made an excellent video showing what an areaportal does if you want to know more.
Unfortunately, this method of optimizing has become rare in a lot more largely used maps and ALMOST ALWAYS are missing in maps that have been decompiled(I'm looking at you, rp_downtown). It's also probably why any addon that allows you to blow doors off their hinges don't open areaportals when it happens(How would creators know this if they don't exist?), since they delete the actual door and replace it with a prop to give you the illusion that you are blowing the door down.
And a small tip for y'all addon creators out there, if you want to make a proper door busting script, you should fire an open input on the door before deleting it.
