it occurs when I select GreenPath (Hallownest mod, act 1) and "leave" button in Custom mode.
This occurs when Keyboard Shortcuts mod puts.

All I can say about it is that it uses the map generation RNG. Could be that it's something in relation to that.
This time I Abandoned, but I do wonder if I'd gone to the regular Act 4 of Downfall if it would have had the same issue or played out normally.
