Did you press 6 ? 6 is the default key to spawn a npc right on your location, and if nothing is done in time, he will just run away

Such a st*pid thing to implement. I have more belt slots, I was selecting my pistol wondering why the hell NPC's started spawning everywhere until one shot me dead.

Options -> key bindings -> NPC Interaction -> Click "Spawn Random Wild Survivors" -> "Assign No Key" -> apply the changes. Now it should be disabled (worked for me).
