Also doesnt work with deactivating it.
Mine was somehow set to <UITogglePosition xsi:nil="true" /> which was making it invisible. There could also be <x> and <y> coordinate values in between opening and closing <UITogglePosition> tags that might be putting the button off your screen. Either way, change whatever is there to <UITogglePosition /> and save.
Next time you load, it will reset the icon position to the top left corner of the screen.
NOTE: You need to enable 'Hidden' files in your windows explorer to be able to see AppData in your "Users\<name>\" directory. For most Windows installs, "Users\" is on the root of your C: drive, but it could be in a different location, depending on how Windows is installed on your PC.
This will cause the icon to appear in the top left corner (that is position 0,0). You can then use the right mouse button to move it wherever you want.
I had a toolbar installed that allowed shrinking the toolbar before I moved back to Yet Another Toolbar. This placed the icon for Theme Mixer somewhere it could not be seen when the toolbar returned to 100% size.
