How do I use this in game? Is it only certain types of servers which it can be used on?

Its a mutator, unless the server is running the mutator, it will only work in single player. You could try hosting a local game with the mutator turned on and then when people join you, they would have it aswell.

I'm having the same issue, except it does not show up on my list of available mutators. Any other things required to use this?
