Will the plague happen in the first bookmark?
I've been playing for around 3 or 4 generations now starting in the first bookmark as the count of Nethander Stead. The Dark Portal has been closed, The Qiraj(Black?) Empire (probably wrong on the name I wasn't paying attention to Kali) invasion happened, and the Burning Legion was defeated. I was wondering will the Plague ever fire? Lich King? Undead Horde stuff? Or do you have to start in the second bookmark for all of that to take place?

The plague has to be triggered by the Lich King himself

It can happen in the first start date but...

The plague can be prevented if the correct decisions to stop Nerzhul are picked in the Beyond the Dark Portal event chain.

Plague happens if Ner'zhul spawns. If you don't want it, start with the 1st bookmark as the scourge invasion option turned off. No matter how many times i tried, Ner'zhul always escapes otherwise and if not Arthas a random character (Tirion in my case -_-) will destroy Lordaeron.

That makes sense now, sadly it was my first playthrough and pretty epic. I have no doubt Ner'Zhul died or something in some ck2 related manner
