I know there's a mod that implements anxiety kind of like this.
In terms of getting it out there to begin with I feel like it'd be cool if maybe certain traits had a higher chance of spawning with them in the first place? It stands to reason that beautiful pawns would get more nookie?
Make them so they appear on the wild on new pawns, some of them hidden in early stages, make new procedures to diagnose them. And make them actually sexually transmitted.
RJW mod has a robust framework on those kinds of diseases, and it also has the option to toggle on/off getting those diseases randomly.
