As for the other... this occurs right after you restart Gmod after a fresh installation.
This is unfortunately an error coming from the addon itself, and the only way to fix this (For now) is launching Gmod, uninstalling and then reinstalling the addon within the same launching client.
Instead of downloading it via Workshop, you should instead manually download it as a zip file. Then you should extract it to your game file in the addons folder (for example: C:\Steam\steamapps\common\GarrysMod\garrysmod\addons). That way, the eye mask on the Octopus should remain without any issues.
