I mostly find it in Towns and villages, check houses, bus stations, parking lots etc. It also depends from the "loot economy" which is controlled by server admins, basically admin should be able to put a higher/smaller % spawn chance. (As for all the other items in the game)

Yeah, mostly it's packed. Box is a white square with some text on it. There might be a model somewhere here.
