How to keep playing
I never saw how to continue play after 3rd act. Does this only work on on first 3 characters? Is there a settting or key I missed. Config, I have it checked. I was playing a new character. Thanks for any response.

It shows up on the map in every map. The icon is similar to the elite icon, but distorted (you can clearly see the difference, but at first it's not very obvious if you don't know)

I do not get the quest for an infinite run. I even killed 2 Nightmares once... :(
