The US Army has been putting American flags patches on uniforms since WWII, though only in special circumstances. It wasn't until sometime in the 90s were they a permanent thing on BDUs for garrison wear. The didn't wear American flag patches on woodland BDUs in the field since it didn't blend in. They only started doing that with ACUs when they came out with subdued patches. Division patches on uniforms was also around at least since WWII for certain units, though it didn't become a regular thing for the Army until around the Vietnam war. It's worth noting that the Marine Corps and Navy don't wear division patches and for all intents and purposes don't wear the American flag patch. Yes, the Marine Corps did briefly wear them in the 80s on certain operations, but generally they don't wear American flag patches.
