This has a lot of potential
"This seems pretty low quality." That was pretty much my first reaction. After all, it just removes the eyes and mouth which most graphics configs remove anyway. But wait! Those configs just leave the eyes as black sockets and don't remove them entirely. Maybe there is hope for this.

Heavy has the biggest personality of all the mercs. He is big, he is loud, he is excited. Remove that and you create a dramatic contrast. For a simple concept, it is one of the most creepy heavy cosmetics Ive seen.

The mouth is what I have problems with. I assume this removes all of his facial expressions since he has no mouth. However, Heavy still speaks in game. That isn't something you can remove. Unless you have the voices from the deep spell he is going to go on yelling just like normal which will break the effect completely. With some black cosmetics and the sound turned off this would be awesome. Maybe the Tomislav also since it is quiet.

Look! I just wrote a huge amount analyzing a joke cosmetics! I bet you didn't read it all! I bet that if you did, you don't care!

kinda like the christmas bell misc this should cause heavy to occasionaly scream HELP ME! in a mufled voice so its even scarier

Ya, it should apply a muffled affect to the heavy, or somthing else, idk

They could also try to make the heavy mute when wearing this item, but that would be unlikely because it would take development time and that time could be used for something else.

i think muffling him on some voice commands could work

I think changing his voice like the halloween spell will work. That makes the voice deeper and somewhat quieter and might seem like it is coming from tha area around the heavy and no his mouth.

have a voice filter effect similar to the engineers fly cosmetics, except his voice lines are muffled.

A muffled effect would be excellent, however unless it's presented alongside the cosmetic, it won't be added. Y'know how adverse Valve are to work nowadays...
