It's been done with the "Flair" item before, shouldn't be that bad.

I imagine it'd be possible to add decals but Valve seems to be iffy about adding items with decal functionality after the original batch. Several items have been added with potential decalibility (sometimes with the functionality being added to the item already) but don't let you do it for some reason.
