I don't think this would look good for that, as the handle is taken from the rest of the weapon so it looks like the medic is wearing it on his arm. I do not know this for sure, however.
I checked the files, and I am not certain if this would work at all, but perhaps if you went into
...\Sisters Sting Mod\Sisters Sting Mod\Ubersaw\tf\models\weapons\c_models\c_ubersaw (or whatever weapon) and changed every instance of ubersaw to Backscratcher then did the installation process normally, that might work.
Please do this at your own risk, however, as I am not certain of the effectivity of this process. I would reccomend backing up your Materials and Models folders if you attempted this.

I could really see this becoming either a Spy knife, Medic saw or an allclass (maybe excluding engi and spy) weapon. It just needs some model fixes to smooth out the clipping issues. This owuld definitely make a really cool spy knife, though.
