I've noticed that when she dies if she has between 100 to 199 points on her star, she goes into what's called as her Neutral form, where she does significantly less damage, has only 1 attack, and has far less time in this form.
Her undying form is available at 200 points or max. After going into either form your star would say like 9999, that doesn't mean it's full, in fact relogging into the server shows it to be near 0. But since it says 9999 your mod thinks that it's ready for use (at 100 or 200) but in reality it's not, so when you transform and use that 9999 it resets your counter to 0 (because that's what it actually was) and because she dies at 0 to go to her soul form, that will explain the repeated dying.
Simple answer: It's a bug, relog to fix it.
