I've got this issue too. Not sure why...

I just restarted it and found a mod was still active {The chezburger tank mod} It works now

Just reopen the game to play i find it the simplest way

It has nothing to do with the map, you can easily fix this witout removing mods by vertifying game cashe.

I'm probably getting the same problem here, but instead tries to enforce concistency on the hunter model. I've tried starting the game with and without other add-ons, and only managed to play the campaign once, whilst other addons were active. Can someone please find a solution for this?

I found out I had the chease burger tank still on, it's files said it was "Hulk."
I finished the problum by far now.

Divaulti, I had the problum useing the mobs pack, where the ghast is tank, enderman is smoker, and the hunter is slime, I found it as a problum you just need to re-install the mobs pack. If that wont work I would edvise looking for some more profecinal help. {Sorry it took so long I have been having problums with work and had to fix it.}
