Trains load wheels incorrectly upon loading save or dupe.
Just putting my comment in a discussion, because it might need some community effort to fix:

There's an error with some trains where the wheels clip into the body upon loading from a save. I know it happens with Percy and the GE C39-8, where the wheels duplicate as well. Hopefully it can be fixed.

I'm can't be sure, but it think it has to do with CBH base creating errors in the Lua. So when you reload the save it messes up the wheels on the engines and cars. As for fixes I couldn't tell ya. I don't know if the mod author is still updating this mod or actively fixing bugs in it.

Did you have the "drive-able" Pirate Ships mod installed?
I found out the hard way, that it tends to cause that sort of problem with all the CBH engines.
