When assigned a squad, combines and metropolice of Half-Life 2 and Garry's Mod have a behavior where only two members of a squad can shoot at once. This doesn't seem to take into account line of fire, so if a squad is separated it can be the case that the squad members you are currently facing don't have the shooting 'permission', but those that do -- can't see you.
Otherwise, some other addon might introduce an issue, if you have addons that modify NPC behavior.
With regards to citizens attacking you -- the system is a bit clunky, but generally you have a bit of time to kill an alerted individual or two before the whole city is alerted. I wish it was more refined, and people would become alerted naturally, but it's the best I could come up with, with my experience and the way the game is structured.
