Damn, I guess I never saw that since they only wear that stuff with pollution around. I'm going to have to note that one down and look into fixing that.
Just did a test with TSIND Power Armor QA disabled and Sealed Power Armor tweak turned on. NPCs still spawn with power armor helmets and gas masks. It's especially off-putting when stellic guards spawn with gas masks.
It's sort of intentional. The idea is you can wear masks over things including closed helmets although it gets a tad silly when you use say a face mask or gas mask. Since it uses layering I don't think there's a good way to prevent it unless you also want to disallow masks entirely for specific items. Also since I use the layer EyeCover its possible there might be items using that layer that would make sense to use together which would cause issues if I block it.
Are your people just being dumb due to outfit restrictions or are NPCs showing up wearing them together? If its NPCs are you using the Sealed Power Armour tweak at all that gives closed helms and armour tox resistance?
Weird, that shouldn't happen. That kind of error seems like it'd be some sort of code weirdness. I actually just tried the two mods together quick to test and I didn't get any errors. Maybe have steam try to verify your game files? Maybe there's something funky there.
No other mods or dlc loaded except of Harmony and XML extansions. No red strings, just annoying message when playing with these two great mods together :(

The tweak Better Quest Rewards patches the def Reward_ItemsStandard to add specified items to the filler options section of quest reward loot. More than likely the mod is patching the same def and adding its own table to the main quest loot selection. You'd have to check what its actually using for criteria to have items in the table, it could use tags or it might be naming the defs.
