Depends on the era. As it's affiliated with the Old Republic and the Sith Empire.

Since this mod is based on KOTOR and SWTOR eras, it's Sith, not Republic.
It's not all that uncommon for ships to change affiliations between factions. We've seen it in Legends with the Republic-Empire-Rebellion several times, and even in Canon with the Empire and Rebellion.
If I recall correctly in Legends, the Venator Class Star Destroyer went from Republic to Empire then to the Rebellion. And the Gozanti was Imperial before some were captured by the Rebel Alliance. Heck, even an Arquitens went from Republic to Empire to Imperial Remnants to Mandalorian.
Sorry for the wall of text. But tl;dr the Centurion in this mod is Sith, not Republic, given the Era. Wookieepedia is reliable.
I'm sorry for the last time we met on these mod pages. I'm hoping we can try again, and be friendly this time around.
Yes, you pick the shipset in empire creation when you start a new game
