I believe this is caused by a conflict in the steam workshop thumbnail image size - specifically, the file size of whatever thumbnail you're using as the workshop thumbnail. Steam doesn't allow you to use one that has a filesize of more than 1000kb, and if you do, it generates that blank template. Just resize your thumbnail image until it's less than 1000kb and it should work.
